My DD-214 (with some personal information removed to prevent identity theft) is provided as verification of my Vietnam combat service. It also includes that portion of my commissioned (officer) active duty service, which commenced May 1969 as a 2nd Lieutenant (2LT), indicated as "Date of Entry."
My earlier service as an "enlisted" soldier prior to my commission as a 2LT [Private (E-1) through Officer Candidate (E-5)] is not shown on this form. My enlistment into the "active duty" US Army commenced 11 June 1968.
Additionally, my Independent Ready Reserve (IRR) and active reserve duty following the issue date of this DD214 are not included as well. My service as a US Army Reserve officer ended in 1990 with the rank of Major.
The following are the definitions for some of the abbreviations used in the form:
ARCOM = Army Commendation Medal
BSM = Bronze Star Medal
CIB = Combat Infantry Badge
CPT = Captain
FA OPER-INT AST = Field Artillery Operations Intelligence Assistant (this is Advanced Individual Training prior to my acceptance to Officer Candidate School)
INF = Infantry
INF UNIT COMDR = Infantry Unit Commander (this designation -- "1542" -- includes infantry platoon leader)
JUNGLESURSCH = Jungle Survival School (Fort Sherman, Canal Zone, Panama)
NDSM = National Defense Service Medal
O/S Bar = Overseas Bar (issued for every 6 months of overseas duty; worn on dress uniform sleeve)
TUSA = Third United States Army
USA INF SCH-INF OFF CAND = US Army Infantry School - Infantry Officer Candidate
USAR = United States Army Reserve
VCM = Vietnam Campaign Medal
VSM = Vietnam Service Medal
